About the role

We are seeking a compassionate and organised Support Coordinator to join our team in Western Sydney, NSW on a full-time basis. This is an integral role within our community services organisation, where you will act as a vital link between clients and the support services they require. As a Support Coordinator, you will be responsible for assessing client needs, developing individualised support plans, and ensuring clients receive coordinated, person-centred care. This role offers the opportunity to make a meaningful difference in the lives of vulnerable individuals within the aged and disability support sector.

What you’ll be doing

• Coordinating and arranging appropriate services and supports from multiple providers to meet identified client needs

• Building and maintaining positive relationships with clients, their families, carers, and service providers

• Monitoring the effectiveness and appropriateness of services provided, making adjustments as required

• Maintaining accurate and detailed client records and documentation in compliance with regulatory requirements

• Advocating on behalf of clients to ensure they receive timely and appropriate access to services and support

• Communicating regularly with clients and stakeholders to provide updates on service coordination progress

• Identifying and responding to client risks or concerns, escalating issues appropriately

• Supporting clients to navigate the broader support system and maximise their independence and wellbeing

• Contributing to team meetings and participating in ongoing professional development activities

What we’re looking for

• Relevant qualifications in social work, disability support, community services, aged care, or a related discipline, and/or equivalent industry experience

• Experience working in a support coordination, case management, or similar community services role

• Strong understanding of the disability support sector, including relevant legislative frameworks

• Excellent communication and interpersonal skills, with the ability to work effectively with diverse individuals and groups

• Highly organised and detail-oriented, with strong administrative and record-keeping abilities

• Ability to work independently and as part of a collaborative team environment

• Sound judgment and the ability to identify and respond appropriately to client risks and concerns

• Current or willingness to obtain a Working with Children Check and/or Working with Vulnerable People Check as required

• Ability to travel to attend client, stakeholder and service provider meetings as required, including interstate engagement where necessary

• A genuine commitment to person-centred care and improving outcomes for vulnerable clients
